Landmark 188 · Trial in people

NEU-411 (NEULARK)

Will tell us whether blocking LRRK2 helps when people are picked by a test, not only by diagnosis. It uses a phone-app score at one year.

In short

After BIIB122 failed in unselected early Parkinson's, Neuron23's 150-person Phase 2 gives a second LRRK2 inhibitor only to people whose disease a companion test flags as LRRK2-driven; the main phase is due to finish around December 2027.

The research record

This is the project's file for this landmark, as written and checked. Sources are listed at the end; anything not yet confirmed is marked "Not yet verified".

Who

Neuron23 (US). Registry: NCT06680830. Started 17 January 2025; recruiting, status last verified August 2026.

What is being tested

LRRK2 is the gene that landmark 7 tied to Parkinson's. The large LUMA trial (landmark 145) found that a LRRK2 inhibitor hit its target yet did not slow people picked only by diagnosis. NEULARK bets that the answer depends on who is treated.

  • About 150 adults with early Parkinson's, picked with an investigational companion diagnostic test. Neuron23 says up to 30% of people have LRRK2-driven disease (company claim; I did not verify it).
  • NEU-411 30 mg once daily or placebo, 1:1, for 52 weeks, then an optional 26-week open-label extension.
  • Phase 2, fully masked.

What would count as success

The main measure is the change in a digital score from Roche's Parkinson's smartphone app (v3.0) at 52 weeks versus placebo, plus safety. Registry estimates: primary completion December 2027, completion June 2028. Neuron23 guides to top-line data in 2027. A positive result would show that selecting patients by LRRK2 biology rescues the approach; a negative one would add to LUMA.
