Landmark 293 · Trial in people

BHV-8000 (TYK2/JAK1 inhibitor)

Will tell us whether an immune-calming pill delays worsening of everyday movement over 48 weeks, in 550 people.

In short

Biohaven is testing an oral drug that blocks two immune-signalling enzymes (TYK2 and JAK1) in 550 people with early Parkinson's, with time to everyday-function worsening as the main measure.

The research record

This is the project's file for this landmark, as written and checked. Sources are listed at the end; anything not yet confirmed is marked "Not yet verified".

Who

Biohaven (the registry sponsor is Biohaven Therapeutics Ltd.).

What happened

  • Mechanism. Biohaven calls it a highly selective, brain-penetrant TYK2/JAK1 inhibitor. TYK2 and JAK1 relay inflammatory signals inside immune cells (including type I interferon and Th17 T-cell pathways). Biohaven says it may modulate immune dysregulation linked to neurodegeneration. The registry text says only "efficacy" and does not name the mechanism.
  • Trial (NCT06976268). Phase 2/3, randomised, quadruple-masked, 10 mg or 20 mg or placebo. Planned 550 people aged 40 to 80 with probable idiopathic Parkinson's with onset within 2 years. Smokers are excluded. 48 weeks double-blind, then 48 weeks open-label. First patient May 2025 (company release, 11 August 2025: global trial, about 185 sites in 13 countries per a secondary report).
  • Measures. Primary: time to the first qualifying worsening on MDS-UPDRS part II (everyday motor function). Secondary: part III, CGI-S, dopamine-transporter scan change, PARCOMS-Function, safety.
  • Phase 1. The company reports no serious adverse events and lower inflammation markers (IP-10, hsCRP, IFN-γ); I could not read the primary data. A mouse efficacy abstract was presented at AAN 2025.
  • Status. Recruiting; registry primary completion August 2027, final September 2028.

Why it moved us toward a cure

Parkinson's patients' T cells react to synuclein fragments (landmark 38), and activated microglia sit in the nigra (landmark 52). BHV-8000 is a test of whether damping those immune signals changes the course. It is built as a pivotal trial, with a DaT-scan secondary measure.

Outcome

Pending. No efficacy data. Note: a part II endpoint also responds to symptomatic effects.
